APPLICATION RESPONSIBILITY

I herby assume all responsibility for any and all risk of property damage or bodily injury that I may sustain while participating in any voluntary effort, or other activity of any nature, including the use of equipment and facilities of Frontline Mission. I currently have no known mental or physical condition that would impair my capability for full participation as intended or expected of me. I certify that the information I have provided in my application to be true, correct, and complete to the best of my knowledge.

FRONTLINE MISSION OATH OF CONFIDENTIALITY

Board members and contract, temporary, casual, part time and full time employees, trainees, and volunteers will hold in strictest confidence any information they obtain about patrons/staff/volunteers or each other while working or volunteering at Frontline Mission. This applies to all information one might obtain in a written, audio or visually recorded format, verbally or through observation.

Frontline Mission requires board members/employees/trainees/volunteers staff/ volunteers to obtain patron's written consent before sharing his/her information at conferences or workshops outside Frontline Mission.

Board members/employees/trainees/volunteers staff/volunteers must disguise Frontline Mission information they use for teaching purposes; patrons and trainees must provide written consent for anyone to use such information revealing the identity of said patron(s)/trainee(s).

Further, board members and contact, temporary, casual, part-time and full-time employees, trainees, volunteer staff, and volunteers will hold in strictest confidence all Frontline Mission financial and/or internal operational information, unless they receive permission from the Chairman of the Board to disseminate such information.

Breach of confidentiality may result in disciplinary action, up to and including termination of employment/volunteering.


FRONTLINE MISSION STATEMENT OF BELIEF

As a Christian humanitarian organization, we feel it is important to protect the beliefs that form the identity of Frontline Mission. It is in this spirit that we require all our volunteer staff members to refrain from engaging in any activity or promoting any beliefs with our patrons or volunteers that contradict our Statement of Beliefs. We recognize and respect that many of our volunteers and patrons do not share a common doctrinal position as us. This is not a condition for acceptance at Frontline Mission. However, when someone acts on our behalf as a staff member, we must be confident that he/she accepts and works within the foundational beliefs on which this ministry is built. Frontline Mission holds to be true the following beliefs, which are common to Christian worldwide:

- We believe in one God who has expressed Himself in personal ways, We know God's personality as Father, Son and Holy Spirit.
-We believe the Bible communicates God's saving message and is our reliable authority in all matters related to faith Christian living.
-We believe all humankind struggles with a download pull.
-We believe that unless we experience God's help and salvation through a faith commitment to Jesus Christ, our future is one of eternal from God and all that is good.
-We believe the love and grace of God is extended to every person in the provision Jesus Christ made through His death and resurrection. By turning from our own selfishness and trusting Jesus Christ, we experience a new life, free from the old patterns of self-destructive acts called sin.
-We believe our new life in Jesus Christ is the gateway to transformation. Following our moment of faith in and acceptance of Jesus, we believe there is another moment in which we present our life completely to Christ in order to live under His authority and experience His transformation power.
-We believe the Christian experience is more than a change of mind. It is accompanied by assurance grounded in both the Scriptures as well as the inner witness of God's Spirit.
-We believe all who live for Jesus Christ have an eternal future. Our belief is such a future includes both the fulfillment of God's creative purpose as well as the accountability of all persons endowed with the power of choice.

FRONTLINE MISSION ACKNOWLEDGEMENT OF HANDBOOK & PROCEDURES.

I have read and understood the Frontline Mission handbook and procedures. I also understand that volunteer positions are assigned by Frontline Mission Staff/Supervisor based on availability of work, and will be rotated to allow for a new volunteer to be scheduled. I also understand that volunteers are scheduled by Frontline Mission Staff and that volunteers do not assign themselves or others volunteer positions. I agree to contact my lead supervisor via phone or Volunteer Local if I cannot make my assigned volunteer commitment as soon as possible. The volunteer relationship between myself and Frontline Mission can be terminated by either party for any or no reason or cause with or without notice.
